Jo, I agree with you. We learned this as Hasta Padangusthasana II, where Hasta Padangusthasana I is with the leg straight ahead.

As I understand it the model it actually demonstrating Parsva Utthita Hasta Padangusthasana (leg to the side) I would expect Utthita Hasta Padangusthasana to be shown with the leg lifted straight to the front. Certainly in the UK Iyengar yoga teachers syllabus the two poses are treated separately (even though they are clearly very closely related)

I can't help but notice that the instructions tell you to grab the outside of your foot, not your big toe, which is also shown by the illustration. Explanation, please?

You don't have to ALWAYS start on your right side, it is up to the individual or the instructor and it's nice sometime to switch it up (seeing how yoga helps with balance starting on the same side always can throw some people out of balance). Also, there are different levels that people are at in yoga so one should not put down another individual just because they aren't holding their toe in this pose. That is simply where their body will allow them to go.
